چکیده

A major challenge in planning and performing resective surgery in drug-resistant epilepsy is the accurate identification of the epileptogenic zone. Currently, intracranial EEG is considered to be the most reliable preresective functional localisation method. Our objective is to non-invasively localise the epileptogenic zone using simultaneous high-density EEG source imaging (ESI) and fMRI of interictal and ictal discharges. We performed two 128-channel EEG recordings, in a shielded room and simultaneous EEG-fMRI, of a focal epilepsy patient in a single session and captured interictal discharges and one seizure exhibiting oscillatory activity of 14 Hz. The locations described by fMRI and ESI of interictal events in the MR scanner and shielded room show good anatomical agreement with each other; the ictal ESI; and with video EEG monitoring, PET and ictal SPECT. Averaging of ictal oscillatory periods strengthens the localisation, while the quantity of concordant interictal events supports reliability of interictal localisations. Simultaneous high-density EEG and fMRI acquisition non-invasively provides added localising information in a multimodal context. Concordant simultaneous ESI and fMRI localisation of interictal and ictal events shows promise in improving noninvasive presurgical evaluation of epilepsy.

EEG-fMRI study of the ictal and interictal epileptic activity in patients with eyelid myoclonia with absences.

PURPOSE To investigate the blood oxygenation level-dependent (BOLD) signal changes correlated with ictal and interictal epileptic discharges using electroencephalography-correlated functional magnetic resonance imaging (EEG-fMRI) in patients with eyelid myoclonia with absences (EMA) and then to explore the pathophysiological mechanisms of epileptic discharges and their effect on brain function....

Funktionelle Bildgebung bei verschiedenen epileptischen Syndromen

Epilepsy is diagnosed when persisting cerebral dysfunction causes recurring epileptic seizures. Distinction between different epileptic syndromes requires a comprehensive clinical workup, including functional neuroimaging techniques. The most established techniques are single photon emission computer tomography (SPECT), mainly used to detect local blood flow changes during ictal activity, and p...

Ictal but Not Interictal Epileptic Discharges Activate Astrocyte Endfeet and Elicit Cerebral Arteriole Responses

Activation of astrocytes by neuronal signals plays a central role in the control of neuronal activity-dependent blood flow changes in the normal brain. The cellular pathways that mediate neurovascular coupling in the epileptic brain remain, however, poorly defined. In a cortical slice model of epilepsy, we found that the ictal, seizure-like discharge, and only to a minor extent the interictal d...

CA3-driven hippocampal-entorhinal loop controls rather than sustains in vitro limbic seizures.

Continuous application of 4-aminopyridine (4-AP, 50 microM) to combined slices of hippocampus-entorhinal cortex obtained from adult mice induces (1) interictal discharges that initiate in the CA3 area and propagate via the hippocampal regions CA1 and subiculum to the entorhinal cortex and return to the hippocampus through the dentate gyrus; and (2) ictal discharges that originate in the entorhi...

Effect of the Entorhinal Cortex on Ictal Discharges in Low-Mg2+-Induced Epileptic Hippocampal Slice Models

The hippocampus plays an important role in the genesis of mesial temporal lobe epilepsy, and the entorhinal cortex (EC) may affect the hippocampal network activity because of the heavy interconnection between them. However, the mechanism by which the EC affects the discharge patterns and the transmission mode of epileptiform discharges within the hippocampus needs further study. Here, multielec...
